Create a layer named "square" and make a colored box. Simply make a box, and brush a few colors in there.
Create another layer named "pattern"
Create and save your pattern
Zoom in and create a 1px black and 1px white line on your "pattern" later.
Select the two lines with your marquee tool.
Define the pattern: Edit -> Define Pattern
Now delete / hide the pattern layer.
Fill layer with pattern
Select the "square" layer which has the colored box.
Use menu Fill Pattern: Edit -> Fill... (Ctrl+F5)
Set settings: Use: Pattern, Mode: Overlay, Opacity: 50%.
Custom Pattern: Select the pattern you have defined.
You can play and tweak these settings for various results.
Press ok, and your scanlines are added.
Make adjustments / tweaking Now we can make our final adjustments to better define our scanlines. In this particular image we are going to adjust the levels slightly. Although this may seem like a barely noticeable adjustment to you, it is actually very important. Every detail, lighting and shade you want perfectioned.
First select your "Square" layer.
Adjust the levels Ctrl+L Or Photoshop Menu: Image -> Adjustments -> Levels...
Now generally you would move the black arrow slider left and right, and get an eye for what looks crisp and sharp.
In my situation, I set the input levels to: 26, 0.82, 255
Your completed image
Now you successfully completed a scanline effect.
Try changing up the pattern for various results. Be sure not to over-use the pattern tool.
